My system had been no improvement on the plant since few weeks ago.
Been keep trying few different way to find the root cause and solution on my system.
The plant grow well until it's have first leave then it slowly dead.
I am using 3" PVC pipe, sponge and coco coir as grow media to see if any difference.
I suspect the problem is due to my LED grow light and water nutrient.
Any advice or other suggestion for my system?

your problem may be a combination of factors. light, too moist/damp medium (causes damping-off) to name a few. lettuce grows best when hardened after germination, before transplanting to your grow medium. Try using a wick instead of directly submerging the base of your net pots.
Thanks Yron. I had another lettuce which placed outdoor and should be less moist compared to indoor.
For the light issue, any other solution for my system?
And is there any sample photo of using the wick you mentioned?

How far away are you keeping those grow lights from your seedlings? They do look a little leggy; they might not be getting enough light.
