I removed some ochro plants from the system because they were seriously restricting light from other plants, but the root systems were so thick I had to cut them out from other roots and not wanting to kill the other plant I unfortunately left a number of dead roots which eventually decayed and caused the crash.

When your system goes anaerobic and kills your fish. Trust me it smells even worse than it looks.
