black spot on gill

I just found one of my tilapia dead. looking at the gills I discovered a black spot on one of the gills, any ideas or thoughts would be appreciated.  My water temp is 78  and the PH is about 7.2 trying to cycle the system  very small amount of Ammonia  0 nitrite very small amount of nitrate.  Thanks
